Please ensure that all sections of the form are completed clearly.

PLEASE NOTE: Please ensure a copy of the site layout diagram is available on site within the method statement to aid accuracy of final test report.

If the Handover Document is not available or not completed or there is any doubt regarding the cleanliness of the enclosure, the 4-stage clearance will not be started.

If a copy of the method statement is not available on site the 4 stage clearance will fail at stage 1, if not available please state the reasons why below:
